New User Application Form


The New User Application Form is available on the Advancement Services website within the Forms button. The form is customized for each institution. The Institution specific PDF form can be filled out and printed for signatures or print the form and fill out the form manually. Please ensure that the information that is handwritten is legible.  Once the form is filled out and signed by the Advisory Committee Representative, it can be scanned and emailed to Advance-requests@usmd.edu or  create a ticket with the New User Form attached or faxed to 301 445 2738. 


The Advisory Committee Representative for each institution is responsible for ensuring the employee completes the New User Information and signs the form agreeing to the propriety of the Advance data. The Advisory Committee Representative is also responsible for completing the Account Information. She/He must ensure the appropriate level of access is requested and that training is scheduled. 


The new user id will be sent within a 24 hour period.  Please ensure all information is provided especially the entity id number and the correct email address. If the email address is incorrect, the user will not receive their temporary password. An email with the user id will be sent to the Advisory Committee Representative. A separate email will be sent to the user with the temporary password.
